Important Note concerning FS-6T’s

If an FS-6T is submitted to our company in order to eliminate a vehicle from a policy or cancel an entire policy, the following shall be used to determine the effective date of the transaction:

A) If the FS-6T is submitted to us within 30 days of the date of the surrender of the plates, the effective date will be the day after the date of the surrender of the plates.

B) If the FS-6T is submitted to the company beyond 30 days of the date of the surrender of the plates, the effective date will be the company’s date of receipt of the FS-6T.
